We have some brief paperwork for you to fill out. It provides us with  general information about yourself  and  your health. Please fill out  new patient paperwork to the best of your ability. 

Dr. Zelony will go over your health concerns with you. He will ask you  various question related to your condition.   The doctor will perform  a  number of specialized tests to determine which areas are affected and  to what extent. 

Your specific condition may require us to take x-rays to either rule out  more serious conditions or assist us the most effective treatment plan  for you. X-rays are done in office to reduce the amount of time lapse  between diagnosis and treatment. 

Once finished with the examination and  all appropriate  studies ,  patients generally will  be provided with their first treatment during  the visit. Treatments include Spinal adjustments, physical therapy and  muscle testing. Since each patient  has their own custom treatment plan, this may or may not include all forms of therapy. 

Prior to leaving, the patient  will be given instructions on what  activities to avoid or what activities to do at home  This may include  application of heat, ice or  what activities to avoid, as well as home  exercise. 

Payment is due at time of service. We accept most insurance and will  handle the paperwork from there. We also have very reasonable payment  options for those patients that do not have insurance or do not have  chiropractic benefits with their insurance
